Patient Admissions
Patients can complete their Hospital Admission Form either online quickly and easily, or complete a printed form for return to the hospital.
How long will it take to complete an online admission form?
It should take around 30 – 45 minutes to complete your admission form and submit it online. Be assured, online admission is convenient and confidential and you can also save the form as a draft at any time if you need to and return to complete it later. You can save your time by booking your admission online.
How long will it take to complete a printed admission form?
It should take around 30-45 minutes to complete a printed form which must then be returned for submission to the hospital. You can get a printed form from your surgeon, or you can download an admission form for printing on this page by clicking on the Download Admission Booklet. The hospital must be in receipt of your Admission form 72 hours prior to your admission time.
What information will I need to complete a patient admission form?
Please ensure the below information is readily available to you before commencing completion of a patient admission form.
- Patient’s personal / next of kin details
- Patient’s Medicare card
- Method of funding (i.e. PrivateInsurance, Self Insured, WorkCover, DVA etc.)
- Current medications and previous health history
You can then opt to complete the information online or complete and return a printed form. See the links for both options below on this page in the section below titled “Two ways to complete your patient admission form.”
Please note: Online forms will not be received by The George Centre until the final submit button is pressed and an acknowledgment is received. Printed forms need to be returned to the hospital 72 hours prior to your procedure admission date.

Please complete your surgical admission form as provided by your surgeon, or go to the Download Admission Booklet on this page to obtain a PDF for printing and completion.
Please note, we must be in receipt of your admission form at least 72 hours prior to the date of your procedure. Your admission form can be returned as follows:
